Feline Idiopathic Cystitis

Feline idiopathic cystitis is chronic bladder inflammation with no identifiable bacterial or structural cause. Also called Pandora Syndrome, it is the most common cause of feline-lower-urinary-tract-disease in young cats (~50% of cases). It is a stress-related neurological disorder where sensitive cats have abnormal hormonal stress responses.

Key Facts

  • Also called FIC, Pandora Syndrome, feline interstitial cystitis
  • Most common urological disease of young cats; uncommon after age 10-12
  • Not a bacterial infection; antibiotics are not effective
  • Affected cats produce higher catecholamines and lower cortisol in response to stress
  • Bladder PSGAG protective lining becomes patchy under stress, exposing tissue to irritating urine
  • Episodes typically last 1-2 weeks; no treatment shortens duration
  • No definitive diagnostic test; diagnosis by exclusion of stones, infection, tumors
  • Male cats at risk for life-threatening urinary-obstruction from mucus/crystal plugs
  • Early life stress (orphaned, bottle-fed, trauma) predisposes cats to FIC
  • Concurrent issues common: obesity, GI problems, over-grooming, anxiety
  • MEMO (Multimodal Environmental Modification) is the evidence-based prevention approach
  • Canned urinary formula diets more effective than dry food
  • Species: cats
